A statewide system enhancing patient safety by monitoring adverse events and supporting quality improvement in Vermont hospitals and ambulatory surgery centers.
The Patient Safety Surveillance & Improvement System (PSSIS) is a collaborative initiative between the Vermont Department of Health and the Vermont Program for Quality in Health Care (VPQHC) that supports strengthening patient safety across Vermont. This system oversees reporting, analysis, and prevention of serious reportable events in hospitals and ambulatory surgery centers.
Under Vermont statute, these healthcare facilities must:
• Report serious reportable events defined by the National Quality Forum
• Track and analyze adverse events and near misses
• Complete root cause analyses with corrective action plans
• Report intentional unsafe acts per regulations
• Maintain patient disclosure policies for adverse events
• Participate in periodic monitoring visits
The PSSIS promotes transparency and continuous improvement by providing structured methodologies for analyzing events, implementing corrections, and sharing best practices to reduce preventable harm.
