Supporting hospitals to identify and respond to food insecurity as part of whole-person care.
Food insecurity, defined as limited or uncertain access to nutritious food, can significantly impact health, contributing to chronic disease, stress, and hospital readmissions. As trusted points of care, hospitals are uniquely positioned to screen for food insecurity and connect patients with community resources.
As part of Bi-State Primary Care Association’s Food Access in Healthcare Consortium, VPQHC launched the _Addressing Food Insecurity in Hospital Settings_ initiative to explore how Vermont hospitals can strengthen their screening and response efforts. The project included a literature review, a statewide survey, data analysis, and site-specific support for participating hospitals.
The initiative concluded with a statewide webinar to share best practices and foster peer learning, featuring Rayna Andrews, nationally recognized food justice advocate and author. A recording of the webinar is available on our Resources page.
This work supports whole-person care by helping hospitals better address the social and nutritional needs of the patients they serve.
