Strengthening healthcare quality through community collaboration and data-driven improvement
The Community and Population Health Care Collaborative supports healthcare organizations in strengthening their approach to community health and population-level improvement. Through facilitated convenings, practice-level coaching, and data-informed technical assistance, this initiative helps providers align care delivery with the broader needs of the communities they serve.

VPQHC works closely with clinical and community partners to improve how practices identify and respond to local health challenges—such as food access, housing instability, transportation barriers, and other non-clinical drivers that affect patient outcomes. The collaborative creates space for shared learning and problem-solving while also offering hands-on support to individual practices to implement and evaluate new strategies.

VPQHC’s role includes facilitating ongoing collaborative meetings, providing data and quality improvement support, and helping document and evaluate improvement efforts. This work reflects our belief that improving population health requires both system-level coordination and local action.

The initiative is rooted in the idea that healthcare organizations, when supported with the right tools and relationships, can be powerful catalysts for change in their communities.
