Collaborative leadership advancing care coordination and patient outcomes across Vermont.
The Vermont Care Management Network, facilitated by the Vermont Program for Quality in Health Care (VPQHC), serves as a statewide collaborative for hospital-based care management leaders. Since its inception in 2014, the Network has aimed to enhance care transitions, and improve patient outcomes through shared learning.
The Network convenes regularly, offering a protected forum for Care Management Directors and Transition Leaders to engage with subject matter experts, discuss emerging challenges, and share best practices.
