Northern New England Practice Transformation Network

The Transforming Clinical Practice Initiative is part of a comprehensive strategy advanced by the Affordable Care Act that enables new levels of coordination, continuity, and integration of care, while transitioning volume-driven systems to value-based, patient-centered, health care services. It builds upon successful models and programs such as the Hospital Value-Based Purchasing Organization Program, Partnership for Patients with Hospital Engagement Networks, and Accountable Care Organizations.  

VPQHC, in partnership with Maine Quality Counts and the New Hampshire Citizens Health Initiative at the University of New Hampshire Institute for Health Policy and Practice, will support 500 primary care and specialist practices across Vermont, Maine, and New Hampshire, to expand their quality improvement capacity, learn from one another, and achieve common goals of improved care, better health, and reduced cost.

VPQHC’s most successful capacity improvement methodology was the Transformation Acceleration Plan (TAP). TAP accelerated the practices through the Transforming Clinical Practice Initiative by identifying and categorizing issues within patient flow; streamlining and standardizing processes; creating increased capacity; and leaving all the participants with a continuous quality improvement mindset.
