Collaborative Expectations:  Medical Home Readiness Collaborative

PURPOSE:  Our goal is to work with teams interested in becoming medical homes by advancing their readiness and maximizing their ability to achieve NCQA standards by the end of the collaborative.

 
What the Participants are expected to do?
  • Create a Collaborative Team click here for more information;
  • Complete pre and post assessment surveys;
  • Establish specific Collaborative goals from among the nine NCQA standards categories (specific standards and minimum number TBD)
  • Use the Model for Improvement Quality Improvement Model and other tools to work on chosen Collaborative goals; 
  • Attend all Learning Sessions, Readiness and Action Period Webinars and Outcomes Congress;
  • Connect with assigned coach weekly for updates and added support; and
  • Report on the achievement of selected outcome measures, including details of changes made and data to support these changes, during learning sessions and at the Outcomes Congress.

     
    What VPQ will do?

  • Facilitate 3 Learning Sessions and an Outcomes Congress to provide evidence-based content information on subject matter, application of the subject matter and methods for real-world process improvement, as well as opportunities for sharing and networking;
  • Provide readiness webinars prior to each learning session;
  • Provide webinar/conference call during each action period to provide additional information and support on specific topics of interest to collaborative participants;
  • Offer Quality Improvement coaching to individual teams;
  • Provide communication strategies to keep teams connected to the Collaborative faculty and colleagues during the Collaborative; and
  • Provide feedback to the teams.
