Q. How does the reviewer pull charts for the NCQA Chart review process?
A. "To determine the 36 patients to review: 1) choose the weekday nearest the date one month prior to the day of the review 2) go either backward or forward from that date, select the 1st 36 patients who had a nonacute care visit and who have any one or more of the 3 clinically important conditions (diabetes, asthma, hypertension)." Q. Do we need to have a Community Health Team (CHT) in place to be NCQA PCMH recognized?
A. No. Although the components of a CHT may help a practice score higher in the NCQA PCMH recognition process a CHT is not essential to recognition.
Q. Do we need to have an Electronic Health Record (EHR) in place to be NCQA PCMH recognized?
A. No. Although the components of an EHR may help a practice score higher in the NCQA PCMH recognition process a EHR is not essential to recognition.
Q. Do we need to have a Panel Manager and/or electronic registry system (i.e. DocSite) in place to be NCQA PCMH recognized?
A. No. Although the skills of a Panel Manager and/or electronic registry system may help a practice score higher in the NCQA PCMH recognition process these components are not essential to recognition.
Q. When do the chart reviews for NCQA PCMH recognition occur?
A. The chart reviews for NCQA PCMH recognition at the time of the survey and submission for recognition to NCQA.
