Standard 6: Element A - Test Tracking - "the practice systematically tracks tests and follows up..." 

Q. What is the reviewer looking for here?  What happens if the practice does not
     have the staffing to call each patient to remind them of scheduled tests but it
     is what is needed to make sure the test happens. 

A. The reviewer would be looking for a spread sheet (electronic or paper) that
     identifies all ordered labs/tests and when those labs/test are complete. The
     clinic also needs to have a procedure clarifying these tracking procedures
     (who is responsible for what) as well as for how they manage/track overdue
     lab/tests/abnormal results as well as their communication/documentation
     plan with the ordering provider/patient/family.  Again, the clinic only needs
     to demonstrate 3 of the 6 types of tracking to pass the element as a "must
     pass".
