Quality Improvement Training and Support to Healthcare Providers

VPQ provides training and support to various physician practices and hospital communities to advance quality improvement and is dedicated to developing a statewide quality improvement infrastructure.  We support quality improvement and clinical best practices at the clinical Microsystems level where care is actually delivered.

Quality Improvement Collaborative
A VPQ QI Collaborative provides a structure to help practice teams identify what they need to work on and gives them the tools to improve.

Current examples are: 
 
 Medical Home Readiness CollaborativeBetween November 2009 and June 2010 clinical practices from around the state will begin or continue their transformation process to become a patient center medical home.  The project is jointly supported by the Vermont Department of Health and the Vermont Rural Health Alliance.              
 
Electronic Health Record Selection Process VPQ will conduct a readiness assessment of primary care practices eligible for VITL (Vermont Information Technology Leaders)  EHR grants from the Vermont Health Information Technology Fund.  The assessment will take place in the fall of 2009 and assist practices to prepare for selecting and implementing information technology as well as informing practices about the practice dimensions that should be addressed to better facilitate EHR adoption.

IT/EHR Implementation Collaborative-- VPQ will conduct a twelve month multi-practice Collaborative for primary care practices that are recipients of VITL health information technology grants.  The curriculum will lead practices through the decisions necessary for selecting and implementing an EHR.  The Collaborative will consist of meetings, distance learning and practice specific coaching. The support is designed to complement the support offered by the EHR vendors and is support by VITL.
 
Critical Access Hospitals
VPQ provides assistance to CAHs with standardizing annual health care quality reviews, preparing for health care quality surveys, selection and administration of small grants supporting quality improvement activities and other locally determined and identified quality issues.
 
Vermont Health Record (VHR)
VPQ provides support users of the VHR, a chronic disease registry tool created for use by Vermont clinicians to help manage treatment plans for patients with diabetes, cardio-vascular disease, and hypertension.
