VPQHC provides training and support to various physician practices, hospitals, and community partner agencies to advance continuous healthcare quality improvement. We are dedicated to developing a statewide quality improvement resource center, supporting evidence based best practices at the clinical Microsystems level where care is actually delivered.

VT Healthcare Associated MDRO Prevention Collaborative
Healthcare-associated infections (HAIs) are infections that are acquired while a patient is receiving medical, surgical, or rehabilitative services in a healthcare setting. Vermont's healthcare community has a long history of working to reduce and eliminate HAIs in Vermont.
Improving Transitions of Care and Reducing Avoidable Readmissions (Collaborative Projects Statewide)
VPQHC has been recognized as the "resource hub" in Vermont to support collaborative learning and partnerships between patients, families, healthcare providers, hospitals, and community agencies aimed at improving coordination of services and reducing fragmentation of care.
Critical Access Hospitals
VPQHC provides assistance to CAHs with standardizing annual health care quality reviews, preparing for health care quality surveys, selection and administration of grants supporting quality improvement activities, and other locally determined and identified quality issues.
