Global Vision
To
improve Health Status for all Vermonters
AIMS
The Vermont health
care system must be safe, patient centered, effective, efficient, timely
and equitable.
STRATEGIC GOALS
-
VPQHC will
expand the application of the Chronic Care Model to other life
conditions.
- VPQHC will enhance its capacity to
produce useful data and analysis to inform and enhance
information-based policy and decision making in the health care
system.
- VPQHC will identify opportunities to
improve end-of-life care.
- VPQHC will identify opportunities to
improve patient safety.
OPERATING PRINCIPLES and VALUES
-
We undertake
projects and engage in activities to achieve the vision, mission and
strategic goals adopted by the Board of Directors of VPQHC.
- VPQHC works to ensure that the
organization’s staffing, resources and financial support are
effectively matched to expected deliverables.
- We actively seek partners and/or
successors to enhance and continue our work.
- We undertake improvement efforts
that are innovative, evidence-based and patient centered.
- We undertake projects that are
ethical and we espouse and adhere to the highest ethical standards
in all work that we undertake.
- We ensure that necessary resources
are identified and in place to support VPQHC activities over time.
- We seek to engage in projects and
activities where the unique strengths and focus of VPQHC can make a
substantial contribution toward quality improvement.
- We make sure aims and objectives of
any undertaking are always clearly defined.
- We make sure that progress towards
aims and objectives is measurable and can be compared to a standard
or benchmark whenever possible.
- We include evaluation and
improvement in everything we do.
- We treat all data and information
with appropriate confidentiality.
- Production and dissemination of
clinical practice guidelines will primarily be undertaken in
conjunction with an implementation activity.
