Vermont Program for Quality in Health Care announces its upcoming Collaborative

 Attached is a brief description of the VPQ Quality Improvement Collaborative and Registration Forms for the October 24, 2008 Kick-Off Session and the November 21, 2008 Learning Session 1. 

The October 24, 2008 Kick-Off session is for:

Who:         Team Leaders and one additional key team member

When:       Friday October 24, 2008 from 9:00 am – 1:00 pm

Where:     Capitol Plaza Montpelier, VT

Purpose of the Kick-Off Session:

The purpose of the October 24 Kick-off Session is for attendees to come away with a clear understanding of the Collaborative and expectations for you and your team over the next eight months.  Participants will be informed of: 

• Collaborative Goals

·         Vision for the Collaborative

·         Overview of Collaborative Structure

• Collaborative Expectations of Faculty, Teams, and Senior Leaders

• Pre-Work Activities for Learning Session 1

Ø  Creating the team

Ø  Defining the aims

Ø  Defining the measures 

Collaborative Sessions are Free of Charge but Registration is Mandatory.

Space is Limited so Please Register Your Team Early. 

Collaborative Dates:

A separate Registration is required for each participant for each Learning Session
