The Vermont Program for Quality in Health Care is excited to announce that we are changing the name of our Collaborative from the Vermont Chronic Care Collaborative to VPQ Learning Community. But it is more than a name change; it is also a change in the learning format that can be used across all clinical settings.

VPQ Learning Community will provide an opportunity for teams that are just starting out, AND those who have participated in previous collaboratives, to work on the continuous improvement of:

• the quality of care for all patients in their practice, and
• the quality of the work environment for all team members.

Using Clinical Microsystems methodology, along with the Model for Improvement (rapid cycle change) and the Chronic Care Model, practices will be provided over-the-shoulder coaching to change their care system.

Who should participate?

Outpatient:
• mental health
• private practices
• palliative, end of life care
Inpatient:
• hospital emergency rooms
• specialty areas

There are several ways for your team to participate:

• attend the one-day Learning Forums (see Save the Dates);
• participate in conference calls (we will be offering three this year) and the list serve;
• sign up to participate in local community training and support.
