VPQ Learning Community LEARNING FORUM Welcome to the VPQ Learning Community – we look
forward to sharing your successes and challenges, and learning from each of you
along the way. In order to make the most of your time during the morning session
of the May 4th Learning Forum in Montpelier, we are encouraging each team to
complete two “prework” assignments. While completion of this work is not
necessary for participation in the Learning Forum it will provide information
that will guide your practice change. There will be lots of time for teams to
work with consultants to analyze their data, choose areas they want to focus on
and plan improvement strategies. The more information that teams can bring with
them, the more opportunity there will be to take advantage of the resources at
the Forum. 1. Gather information about your patient population, including:
Hopefully, most if not all of this information can be gathered through a practice management system or billing service. Give us a call if you want help with how to gather this data. 2. Complete the Core Processes Assessment. Click here to open and print the survey to complete. (This survey has been supplied by the Northeast Health Care Quality Foundation and should be completed by all members of your practice). Completed surveys can be mailed or faxed to: Patricia
Launer VPQ will send you a document containing the tabulated results from your practice. (These results will be based on the number of respondents that actually rated each process.) If you want responses to be anonymous, just make sure staff notes the name of your practice on the survey and have them mail the survey individually to VPQ. Alternatively, if staff is comfortable with having the surveys collected at the practice and sent to VPQ together, that works for us as well. Also part of this packet is the Greenbook* – a tool to help you better understand Clinical Microsystems. (If you are unable to access the Green Book please let us know and we can send you a hard copy.) You'll note that the Core and Supporting Processes Survey can be found on page 19. *
