FREQUENTLY ASKED QUESTIONS

Who should participate?
The Learning Community is open to all healthcare teams interested in improving the quality of care they provide, including:

  • past participants in the Chronic Care Collaborative
  • new teams
  • teams from the Blueprint pilot communities
  • teams interested in applying to become a Blueprint pilot community

Should I attend if I am a Blueprint Pilot Community or have submitted an application to become one?
The Learning Forum is open to all and is designed to support current pilot community teams as well as future ones by providing training and information that supports the Blueprint and readies new teams for the next round of spread.

How is this different from the Blueprint Initiative?
The May 4th curriculum will complement the community based learning available through the Blueprint. The morning session will provide intense exposure to Clinical Microsystems concepts, a core component of the Blueprint initiative. Both existing and expectant Blueprint community teams will benefit. The afternoon agenda is not part of the current Blueprint offerings.

Why should my team participate?
The Learning Forum is an opportunity to network with teams from around the state with a common goal, improving the quality of care provided to patients, particularly those with chronic illnesses. Some of the participating teams have been part of the Chronic Care Collaborative in the past; some teams are just beginning to learn about the Model for Improvement, the Chronic Care Model, and Clinical Microsystems. Through listening to scheduled speakers and participating in over-the-shoulder working sessions, teams will be given the chance to analyze their practice and put into place practical, doable improvements. (What can you do by next Tuesday?) Time will be scheduled for teams to share successes and challenges, learning from each other as the day progresses.

What does a team look like?
A team can consist of a provider (doctor, nurse practitioner, physicians assistant), a nurse, and administrative staff. Some teams also choose to include additional team members including dieticians, physical therapists, self-management educators, and patients.

Do I have to come as part of a team?
We encourage everyone to attend as part of a team whenever possible; however, we understand that this is not always feasible. Teams who have attended Learning Sessions in the past have felt very strongly that the sessions allow much needed time away from the work setting to focus on what they would like to improve. If you attend without a team we will try to pair you with working teams.

Can I limit my participation to attending the Forums and occasional conference calls?
Definitely – the goal of the VPQ Learning Community is to provide diverse opportunities for learning and networking. We welcome participation in all or any of the activities offered.

Will I earn any CME or CEU credits if I attend?
The Learning Forum has been approved for 6.25 CME’s and 7.5 CEU’s.
