Improving Transitions of Care | | In order to support the Patient Safety and Affordable Care Act passed by Congress in 2010, VPQHC is managing several VT Office of Rural Health grant funded projects focused on improving transitions of care and reducing avoidable hospital readmissions.
VPQHC has been recognized as the “resource hub” in Vermont to support partnerships between patients, families, healthcare providers, hospitals, and community agencies aimed at improving coordination of services and reducing fragmentation of care.
