PSSIS / Never Events



The Patient Safety Surveillance and Improvement System (PSSIS) was created for the purpose of improving patient safety, eliminating adverse events in Vermont hospitals, and supporting and facilitating quality improvement efforts by hospitals. The Vermont Department of Health is charged by statute to operate the PSSIS, and contracts with the Vermont Program for Quality in Health Care (VPQHC) to administer the System.

National Quality Forum (NQF) considers a serious reportable event to be unambiguous, largely, if not entirely, preventable, serious, and any of the following:

  •  Adverse
  • Indicative of a problem in a healthcare setting's safety systems
  •  Important for public credibility or public accountability


Additionally, Serious Reportable Events (SRE) are events that are:

  • Of concern to both the public and healthcare professionals and providers
  •  Clearly identifiable and measurable
  • Feasible to including in a reporting system
  • Of a nature such that the risk of occurrence is significantly influenced by the policies and procedures of the healthcare facility.
