"EXCEL" Regional Reports Mt Ascutney Hospital Mt Ascutney Hospital and Health Center instituted a hospice and palliative care program five years ago. The Palliative Care Task Force routinely reviews in-patient records after discharge as part of an ongoing CQI process. This review also includes results of family interviews, and staff debriefings. Standard orders and a Care Map were developed, and are in the process of being revised as part of our on-going commitment to quality care. A Service of Remembrance and Candlelighting is conducted annually for staff and the community as a way to acknowledge the lives of all those who have touched us, particularly those who died in the previous year. The Task Force is currently working on planning community outreach and education programs based on the Bill Moyers program "On Our Own Terms", as well as winter programs focusing on Arts and Healing, and Caring for the Soul of the Dying. Central Vermont PALLIATIVE CARE SERVICE MISSION: Provide a service to optimize physical, emotional and spiritual comfort to CVMC patients who are chronically ill or at end of life as well as provide education and support to patient's loved ones and caregivers. VISION: Multidisciplinary, collaborative approach to the care of patients, their families and caregivers. The service will provide pain management consultations; assistance with disease management decisions; end of life decisions; symptom management; placement issues; bereavement referrals; education and staff support.
