SUBJECT: End-of-Life/Palliative Care grant program


The Department of Aging and Disabilities has completed its request for proposals to improve and enhance palliative and end-of-life care for nursing home residents. Thirteen End-of-Life/Palliative Care grant proposals were submitted to the Department for this first-time "end-of-life" initiative. We were able to fund four of the proposals for a total of $60,000. These projects will begin July 1 and be funded for the fiscal year 2001. The grantees are listed below. Many thanks to all the applicants for their hard work.

Bennington Health & Rehabilitation for $7,753
A multi-disciplinary education program for nursing home staff, and a family and resident support group.

Central Vermont for $25,000
Community-wide "consistent" pain assessment and management using scales, tools and protocols for the Central Vermont health care continuum (nursing homes, residential care homes, assisted living facilities, homecare, hospice, Central Vermont Medical Center, physician offices and clinics.)

Orleans-Northern Essex for $15,000
Regional program for all six nursing homes that will: develop and support a common educational program for the staff, develop and train volunteers to provide respite care, provide training for the Palliative Team, and enhance the involvement of physicians.

Stratton House for $12,247
Educational program for staff, physicians, residents and family; formation of a palliative care team; and development of a palliative care protocol.
