 * The SmartQuest Pro Version 4.5 IVR software (Phone Survey and Outbound Dialing software), Intel/Dialogic PCI Voice Board, and standard 1-800 telephone outline provide the necessary equipment to support a fully functioning IVR system with the PC support provided by the Vermont Project for Quality in Healthcare, Inc.  The pricing of this equipment includes a 50% discount from Telesage.  The Standard 1-800 telephone outline was derived from the industry rate of: $300.00 installation, $22.00 per month fee (for 1 month), and $.06 per minute for 15 minutes per individual call at 20 total calls.

 

**  These budget items will be paid for by the Vermont Project for Quality in Health Care, the Vermont Medical Society and other partner organizations.

 

*** Dr. Turner will be responsible for the survey-IVR development, programming/implementation, system validation, monitoring, IRB application, and final analyses of the IVR administration of the ExCEL End-of-Life Survey.

 

* * * * Steve Larose will be responsible for identifying media and outreach targets, developing letters, press releases, op-eds, newsletter articles and other outreach materials to recruit pilot participants and inform target audiences about the full survey
