"EXCEL" Steering Committee Minutes

Present

The meeting began with a discussion of the Hospice and ExCEL meetings at Lake Morey in June. A suggestion was made that next year we should prepare separate announcements of the two meetings because some people found the combined announcement confusing.

Brattleboro - follow-up
Since the meetings a steering committee has formed and has met three times. They have planned training in the hospital and community for August and September. A member of their steering committee who works at Woodbury College has offered the group the opportunity to participate in a 4-day workshop on mediation at no charge.

The new EMT-DNR protocol will be piloted in Brattleboro before being implemented statewide. Jeff Bell MD will be coordinating this.

Bennington
Advance care planning trainings are scheduled for October 13 and 14 in Bennington. Local leaders will be involved including Barbara Fane, an oncology nurse who will do a presentation on hospice.

EMT-DNR -
Next meeting of the EMT/DNR group is Thursday, September 14 at 6 p.m at Central Vermont Home Health & Hospice.

Central Vermont
Central Vermont is marketing hospice training to EMT's. This is designed to help with recruiting volunteers and also to pave the way for the coming EMT/DNR policy.

The Central Vermont palliative care service is off the ground. They have had 4 referrals so far since they opened in July. Primary care docs, specialists and nurses have requested consults. They have two nurses and two docs who offer coverage at all times and so far have been able to consult within 4 hours. The physicians bill regular consults for medical care in the hospital and the nurses bill through a consultation contract they have with the hospital

Central Vermont hospital is also in the process of developing a family comfort room where families can "veg out" and rest as opposed to a "dying room" for patients.

VEN Posters
VEN has "How can we respect your wishes if we don't know what they are" posters available for posting in doctors and other medical offices. VMS will include a notice about these in the next newsletter and/or policy bulletin.

North East Kingdom
The North East Kingdom group is moving forward on their project with nursing homes. They are educating nursing home staff and recruiting volunteers to sit with people who are dying. A dinner meeting with physicians is scheduled for Monday July 24 to review the results of their survey of families of dying patients and to increase physician involvement and participation.

North Country Hospital has agree to fund 1 of their employed docs to become certified in palliative care and provide palliative care in the hospital. (Two other docs in the community, Denise Niemira and Skip Linton are already certified.)

On Our Own Terms - (Moyers' Program)
  • Northeast Kingdom - Discussion groups are planned for the month after the program in several communities. They are promoting the program and the discussions at the Barton Fair, through the Rotary, in the hospital newsletter and there will be copies of the tapes in the libraries and educators will lead discussions in the high school and at CCV.
  • Chittenden County - A media advertising campaign is planned and a hot line will be staffed at the end of each program. On September 19 there will be an open forum at Faith Methodist Church in South Burlington. They are also working with the ecumenical council to provide tapes to churches for viewing in October, ecumenical month.
  • VPR - A suggestion was made to try to coordinate a program discussing end-of life care for Switchboard on VPR.

Letter to hospitals
Steering committee members will feed information to Arnold Golodetz, MD, who will prepare another draft. VMS will then send it out to the hospitals.

Last Acts -
Last Acts has developed some principles of palliative care. Denise will send them to VMS and VMS will distribute them through the list serve and put them on the web site.
