Chronic Illness in Vermont

Chronic Illness in Vermont encompasses a wide spectrum of disease processes including diabetes, coronary artery disease (CAD), congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), asthma, and depression.  The Vermont Program for Quality in Health Care (VPQ) has been using the Chronic Care Model as the model to strive for to provide quality patient care for people with chronic illnesses.  The Chronic Care Model was developed by Ed Wagner, MD, MPH, (Director of the MacColl Institute for Healthcare Innovation, Group Health Cooperative of Puget Sound), and his colleagues during the 1990’s.  The Chronic Care Model identifies six key components of a successful healthcare system:  the community, the health system, self-management support, delivery system design, decision support and clinical information systems.   Success is defined as productive interactions between an informed, activated patient and a prepared, proactive healthcare team.

In 2005, the Chronic Care Model was adopted by Vermont as the basis of healthcare reform in the state.  Titled the Vermont Blueprint for Health, this reform initiative has been recognized nationally as a model for addressing the needs of people with chronic illnesses (www.healthvermont.gov/blueprint.aspx).  VPQ is integrally connected to the work of the Blueprint providing assistance with clinical information technology through the Vermont Health Registry (VHR) and provider training through the Vermont Learning Community.
